Maintain Corporate Account
To be completed and submitted by an Authorizing Officer
To ensure that you can add a new Control Account smoothly, please contact our Corporate Services Center or Sales Representative before filling in the form.
Add New Card Members
You can apply for a new Card by filling in a paper form, or if your company’s liability type is either LL, Corporate or Combined, you can apply online via American Express @ Work®.
Please fill in the appropriate Card Member Application Form for your company’s liability type and submit it to us after it has been signed by the Authorizing Officer.
*Card applicants must submit a copy of official identification document(s) along with the application form.
Log in to @ Work and access “Apply For Card” in the “Program Management” section.
*Enrolment is required to apply for a Card via @ Work. Please enrol here if you have not yet enroled.
Maintain Card Members and Users
To set up or update bank account details, please review the information in ‘About Payment’, complete the relevant form below and submit it to us.

Change registered information or cancel Card
Please submit this form to change registered information or cancel your Card. When a Card Member fills in the form, approval is required by an Authorizing Officer or a Contact Person depending on the type of changes. If you are an Authorizing Officer or a Contact Person already enrolled to @ Work, you can also apply for Card cancellation online via @ Work OPM.

Corporate Card Conversion
To convert Card types (including conversion to the American Express® JR TOKAI Express Corporate Card), please submit the form below. Corporate Card conversion requires prior enrolment by your company. Once enrolment is confirmed, submit the Conversion Form with the signature of your company’s Authorizing Officer.
