アメリカン・エキスプレス法人会員基本申込書等、会社代表者様のご署名・ご捺印が必要な書類をご提出の際は、会社代表者様の氏名に加え、自宅住所と生年月日の申告をお願いいたします。

Please note that American Express requires the name, home address, and date of birth of the Corporate Representative when providing their official authorization to confirm the qualifications for utilizing our products and services.
When you submit any documents that require the signature of the Corporate Representative, such as “The American Express Corporate Account Agreement and Application Form”, please ensure to include the name, address, and date of birth of the Corporate Representative.
