• The specialized rates are applicable only on payment made at payment counter at PHC department of Venkateshwar Hospital Dwarka.
• Prebooking for Preventive Health Packages is compulsory.
• Package inclusions and cost is captured in Annexure 1 here.  Inclusions and cost are subject to change basis management's decision.
• The offer can be availed multiple times during the offer period.
• The offer is also applicable on Supplementary Cards.
• All disputes relating to billing offer, services etc. shall be resolved at the hospital level only.
• There are no blackout dates for this offer.
• The discount will be provided on the billed amount as per Invoice, inclusive of Taxes.

• Offer is not valid on EMI Transactions.

Steps to Avail the offer:

1. Cardmembers will have to write via email to phc@venkateshwarhospital.com or call or WhatsApp at 7290072977 Monday to Saturday (9am to 5pm) for appointment.
2. Venkateshwar Hospital Health Checkup team (PHC Department) will share confirmation on email.
3. Cardmember will visit Venkateshwar Hospital to get the test conducted and pay at payment counter with eligible American Express Card.
